I am enclosing this completed form and entry fee (NON-REFUNDABLE) payable to TBSWV.
In signing this form, I hereby waive and release Traditional Bowhunters of Southern West Virginia and Raleigh County Recreation
Authority, it's officials, agents, employees and sponsors from any and all damages, claims, demands, cost or expenses relating
to injury of any person or property which I may sustain or which I may cause by reason of participating in or in connection
with this tournament. I further agree that I will never sue Traditional Bowhunters of Southern West Virginia and or Raleigh
County Recreation Authority for damages on account of any injury or damages I suffer or cause whether known now
or which may develop in the future. I understand that in waving my right to sue Traditional Bowhunters of Southern West Virginia
and Raleigh County Recreation Authority. I am also waving the rights of recovery from Traditional Bowhunters of Southern West
Virginia and Rraleigh County Recreation Authority of my insurance carrier for anything they may pay on my behalf.

Preregistration cost for fun courses per person is $15 for all
three days. Camping is NOT included.
Preregistration cost for all three days for fun courses, competition and Coon
shoots is $25.
Cost at the gate for fun courses all three days per person is $20
Cost at the gate for the competition course is additional $5. Coon shoots
are additional $5 each night.
All kids 12 and under shoot free with an adult
